JOINT-AFFIDAVIT OF TWO DISINTERESTED PERSONS

We, JOEY D. GEMORANION and JESSA C. JUNTILLA, both of legal age, Filipino
citizen, both married and both residents of Brgy. Cabulihan and Brgy. Linao, Ormoc City,
Leyte, after having been sworn in accordance with the law, hereby depose and say:
1. That we are not related either by consanguinity or affinity to MARIA SHEILA
VELARDE DOYON, of legal age, Filipino citizen, single and resident of Purok 3
Brgy. Cabulihan, Ormoc City;
2. That we personally know as fact that the true and real name MARIA SHEILA
VELARDE DOYON as indicate hereto attached the authenticated copy of his
Certificate of Birth issued Philippine Statistic Authority (PSA);
3. That we have known personally as fact that some documents concerning to her
Transfer Certificate of Title No. TC-1962 her first name was mistakenly written as
MARIA SHIELA instead MARIA SHEILA;
4. That we personally known as fact that the MARIA SHEILA VELARDE DOYON
as reflected on Certificate of birth and the name MARIA SHIELA VELARDE
DOYON as reflected on her Transfer Certificate of Title as mentioned above is refer
as same one person;
5. That we are executing this affidavit to attest the truth of the foregoing, and for
whatever legal purpose this affidavit may serve.
