400104 do hereby solemnly declare and affirm as under :

I say that my daughter BHAWANA KUMARI SUMERMAL PUROHIT is

studying in X Std. Division A, Roll No.3, Reference No. MSS 12-472, G.R. No.

10077 at Dr.Sarvepalli Radhakrishnan Vidyalaya, Malad West, Mumbai.

I say that in the School Record my daughter’s name is recorded as a BHAWANA
KUMARI SUMERMAL PUROHIT.

I say that I have corrected / changed my daughter’s name in Maharashtra

Gazette Publication vide Gazette No. M-21126304 and as per Gazette records
my daughter’s new changed/corrected name is BHAVANA SUMER PUROHIT

I say that both the names BHAWANA KUMARI SUMERMAL PUROHIT and
BHAVANA SUMER PUROHIT are belongs to one and the same person i.e. my
son

I am making this Affidavit to produce the same to the concern school authority to
enter my son’s changed/corrected name in her school record as BHAVANA
SUMER PUROHIT.

Whatever stated above is true and correct to the best of my knowledge and
belief.
