AFFIDAVIT OF NON-FILING OF INCOME TAX RETURN

I, PRECIOSA C. SALVAME, married, of legal age, Filipino and a

resident of Brgy. Alangilan Batangas City, Philippines, after having been duly
sworn to in accordance with law, hereby depose and say, that:

1. I am the mother of HARVEY JASPER C. SALVAME, a second

year Bachelor of Science in Business Administration Major in Marketing
Management student enrolled at St. Bridget College, Batangas City;

2. Likewise, HARVEY JASPER C. SALVAME is a grantee of the

Tulong Dunong Program scholarship provided by the Commission on Higher
Education;

3. I am a self-employed individual with an annual income not

exceeding P250,000.00 and I am not employed by any employer;

4. Considering the foregoing, I have no taxable income to declare, thus,

have not filed any Income Tax Return before the BIR Office;

5. I am executing this affidavit to attest to the truthfulness of all the

foregoing facts and for any legal purpose/s it may serve, such as but not limited to
the request from the BIR Office of the issuance of the appropriate Certificate of
Tax Exemption and the grant of the said scholarship to my son HARVEY
JASPER C. SALVAME.
