Explanation of the handling of personal information acquired for health

confirmation utilizing the LINE app, etc.

Ministry of Health, Labour and Welfare

To help ensure that everyone can live safely and securely during the outbreak of the new type of coronavirus, we are
confirming the health condition of everyone who returns to Japan from overseas during the 14-day period after their arrival
by having the public health center or other organization in the municipality where they live call or otherwise contact them.
To reduce the burden this imposes on everyone, we are cooperating with LINE Corporation so returnees can use the
LINE app (or phone calls by an AI) Chats screen (or automatic voice calls) to respond instead of having public health
centers directly call everyone to confirm their health condition after they return to Japan.
If you would like to use the LINE app instead of receiving phone calls from a public health center to confirm your health
condition after returning to Japan, you must first agree to the contents of this document. Next, fill in your address, name, and
the date at the end of this document and then submit the document to the quarantine station staff.

1. Flow for using LINE to confirm your health condition after you return to Japan (“these efforts”)
(1) The phone number, municipality where you live, and return date you indicated on the quarantine questionnaire you
submitted upon returning to Japan will be provided to LINE Corporation. If you have a LINE account, the above phone
number will be used to contact it.
(2) During the 14-day period after you return to Japan—based on the information you provided in (1)—LINE Corporation’s
system will be used every day to automatically send your account a question message regarding your health condition on
that day from the official Ministry of Health, Labour and Welfare LINE account (or you will be asked via an automatic
voice call if you do not have such an account), so please respond. The details of your responses on the Chats screen (or
during the automatic voice calls) will be analyzed by an AI, and, if necessary, we will automatically send you additional
questions or information on a consultation center for returnees and those in close contact with the virus.
(3) Your response information from (2) will be provided to the Ministry of Health, Labour and Welfare through the LINE
Corporation every day. In addition, your response information will be provided to the municipality (public health center)
where you live.
(4) Once a certain period passes after the health-condition confirmation period following your return to Japan, the official
Ministry of Health, Labour and Welfare account will leave your Chats screen, and only you will be able to access this
screen.

2. Range of personal information acquired during these efforts

During the health-condition confirmation period after you return to Japan, the Ministry of Health, Labour and Welfare will
acquire all information related to your health condition and other details discussed by you and the official Ministry of Health,
Labour and Welfare account on your LINE Chats screen.

3. Purposes of using personal information acquired during these efforts

The Ministry of Health, Labour and Welfare will use acquired personal information for the following purposes:
(1) To implement measures to deal with the new type of coronavirus, including confirming everyone’s health condition after
they return to Japan and providing information depending on everyone’s physical condition
(2) To implement nationwide measures to deal with the new type of coronavirus by analyzing trends in the outbreak of the
disease
(3) To provide information to the municipality where you live so that the municipality (public health center) can communicate
with you about the new type of coronavirus or implement other measures to deal with the new type of coronavirus

4. Use of the LINE app, etc.
