Chapter – 1

INTRODUCTION
INTRODUCTION

Indian healthcare market is growing at good speed and presently both online and offline
pharmacies are doing business. The Indian pharmacy market is huge, estimated by industry to be
around Rs 1.2 lakh crore in size and out of that the online pharmacy market, estimated to be a
fraction of that at Rs 700-800 crore, is sufferer by regulatory issues that are now showing signs
of easing.

The growths of online companies are imposing huge threat to these offline companies. As a
result of this offline pharmacies are united together and on October 2015 the retailers shut their
shops as a mark of protest against the online drug retail business. This strike was nationwide and
AICOD (All India Organization of Chemists and Druggists) members took participation in it as
the Health Minister of India did not assure AICOD against their grievances. The main objection
against them is prescription drugs are sold through these online companies without prescription.
The sale and distribution of drugs in the country falls under the Drugs and Cosmetics Rules,
1945. However, the rules do not distinguish between offline and online sale of drugs. As a result,
even after more than seven decades the matter is still debatable.

In India mainly two categories of drugs are available a) Prescription drugs b) Nonprescription
drugs. The objective of the study is to examine and analyze present scenario - of both offline and
online pharmacies. and the Govt. role in this perspective. The advantages of online pharmacies

are privacy, avail great choice, lower prices and convenience mainly. Similarly, the
disadvantages include lack of physical evaluation in place, some do not dispense drugs through
licensed pharmacies, some online pharmacies do not adequately safeguard patient’s health, some
do not adequately protect personal and financial information. In USA both online and offline are
licensed, however online pharmacies are allowed to operate based on certain restrictions. Indian
government must take of patient information privacy and security regarding online pharmacies.
They can educate common people about safe practices of purchasing online medicines. They can
provide list of approved /accredited online pharmacies in future. No doubt E-Pharmacy improves
consumer convenience and access and this will give benefits to elderly patients suffering from
chronic ailments because they need to visit pharmacy to buy medicines. At present there is a lot
of misunderstanding about e-Pharmacies particularly those are doing offline business in this

1
segment. At present the leading pharmacy players are- 1mg, Net meds, MChemist, Myra, Midlife
etc. Today around 150 startups are currently operating as online pharmacy. India needs an
effective regulatory mechanism regarding this aspect. this research focus on the present situation
of retail pharmacy sector of India. Pharma retail industry is sensitive zone because it is related to
patients’ health. Second, we have analyzed the advantages and disadvantages of pharma retail
industry.

 Pharmacy Retail Market in India is expected to reach a value of INR 2.9 bn by 2022.
 The pharmacy retail market in the India has witnessed a growth in recent years on
account of rising demand for OTC drugs and private label products fuelled by extensive
advertisement by various organizations.
 Indian E – pharmacy industry, it is anticipated to grow at a CAGR of over 20%, crossing
the US$ 3 Billion mark by 2024.
 The domestic Indian Pharmaceutical industry is estimated to be worth $ 12 billion,
growing at about 8-9% annually.
 The organized pharmaceutical retail market size is estimated to be Rs.2600 crores.

Conventional Pharmacy

Pharmacists are the health professionals most accessible to the public. They supply medicines in
accordance with a prescription or, when legally permitted, sell them without a prescription. In
addition to ensuring an accurate supply of appropriate products, their professional activities also
cover counselling of patients at the time of dispensing of prescription and non-prescription drugs,
drug information to health professionals, patients and the general public, and participation in
health-promotion programmers.

Pros of Conventional Pharmacy

1. Processing of Prescriptions: The pharmacist verifies the legality, safety and appropriateness
of the prescription order, checks the patient medication record before dispensing the prescription
(when such records are kept in the pharmacy), ensures that the quantities of medication are
dispensed accurately, and decides whether the medication should be handed to the patient with
appropriate counselling.

2
2. Care of Patients: The pharmacists seek to collect and integrate information about the
patient’s drug history, clarify the patient’s understanding of the intended dosage regimen and
method of administration, and advises the patient of drug-related precautions, and in some
countries, monitors and evaluates the therapeutic response.

3. Monitoring of Drug Utilization: The pharmacist can participate in arrangements for

monitoring the utilization of drugs, such as practice research projects, and schemes to analyses
prescriptions for the monitoring of adverse drug reactions

. 4. Responding to Symptoms of Minor Ailments: The pharmacist receives requests from

members of the public for advice on a variety of symptoms and, when indicated, refers the
inquiries to a medical practitioner. If the symptoms relate to a self-limiting minor ailment, the
pharmacist can supply a non-prescription medicine, with advice to consult a medical practitioner
if the symptoms persist for more than a few days. Alternatively, the pharmacist may give advice
without supplying medicine.

Cons of Conventional Pharmacy

1. Time: Sometimes drugs are not available at nearby pharmacy store they may prove to be time
consuming for the elderly and busy scheduled peoples to see proper drugs at right time to prevent
the risk of patient’s health.

2. Price: The price of drugs is more than online pharmacy medicines due to middle man’s
profit. The patient due to its poverty may find difficulty in buying costly and branded drugs
which may prolong its long-term diseases.

3. Inconvenience: Chronic elderly patients who are not able to go to pharmacy store may cause
difficulty in finding the stores situated at long distances or unrelated places.

E- Pharmacy

An E-Pharmacy is a pharmacy that operates over the internet by selling drugs, medicines &
sending them to the customers. Internet making all the things easy & convenient. Patients can

Pros of an E-Pharmacy

1. Convenience: It is easy to order online medicines with prescription. Individuals who live
great distances from a terrestrial pharmacy, the elderly, disabled persons, and those whose daily
schedule includes additional hassles, all can benefit from the easy and fast purchasing of
medicines online. Additionally, shipping charges are often less than the expense incurred from
travel costs associated with a visit to a traditional pharmacy.

2. Future Vision: Under Digital India initiative, Jan Aushidhi Program the governments takes
initiative to create awareness & provide access of affordable medicines to the general public
which compromising to good quality drugs. The operating model of E-pharmacy which has
mobile & web-based applications, directly linked to inventory & dispensing existing at Jan
Aushadhi Stores.

3. Time Saving: In just a matter of minutes, you can log into the internet, head over to an E-
pharmacy website and buy any prescription medicine that you want. It takes less time and effort
than travelling to your local pharmacy and having to wait in line for your medicine.

4. Lower Price: Drug shopping on the internet helps a buyer to save money. Several surveys
conducted by research bodies have shown that as much as one third the price can be saved on
prescribed medicines if they are bought online.

5. Private & Confidential: This is particularly helpful to people who find it hard to speak face
to face with doctors & pharmacists. It is also helpful to people who feel as if they have very

5
embarrassing or shameful situations like erectile, misfunctioning, acne, STD’s, hair loss etc.
They can order their products without feeling embarrassed or judged.

6. Range of Choices: E-pharmacy offers a great variety of options. They usually have larger
stocks of drugs than a physical pharmacy.

7. Drug Approval: Most of these legal stores have a process of drug approval. They require a
prescription from a personal physician, or at least offer a drug as a prescribed medication only
after the consumer fills up a questionnaire. This questionnaire is reviewed by a doctor before the
drug is dispatched.

8. Medical Information: Some online pharmacies provide useful information about

medications and diseases as well as links to medical resources such as universities, government
agencies, and health associations.

Cons of E-pharmacy

1. Lack of Physical evaluation in place: Since the internet makes it easy to sidestep the safety
net, these online stores do not have any processes of medical supervision or physical evaluation
in place. If at all there is any such process, these are not performed by a licensed health
professional.

2. Need of using the same Online Pharmacy all of the Time: If you do not use the same
online pharmacy all of the time, they may not be aware of any potential drug interactions, it can
be difficult to find someone to answer any specific questions about medications you may have, it
is not possible to get the prescription the same day and you lose the personal interaction.

3. Some online Pharmacies don't Adequately Safeguard your Health: By failing to require a
prescription, you could take medication that is not appropriate for you. It could take patient to the
extreme stages of sickness. PharmacyChecker.com- approved pharmacies must require
prescriptions. Some illegal online drug stores deal with dangerous contaminated, counterfeit or
outdated drugs that can lead to dangerous drug interactions.

4. Some do not Adequately Protect your Personal and Financial Information: It's important
for pharmacy websites to publish their privacy policy, one that promises not to share your

6
personal information with third parties. Online financial transactions should be secured through
adequate use of encryption technology. Pharmacychecker.com verifies whether privacy is
promised and transmittal of financial information is secured.

10
CHAPTER – 3
SWOC ANALYSIS

11
Strength

 One of the strengths is that the demand for medicines is stable:

In this pandemic age, only the medical stores are the most stable ones in the market. Almost
everyone needs medicines in one or another way. After all, every house has at least one
paracetamol medicine.

 Comfort

This is one of the main reasons why people are so drawn towards online medical consultation
because of the comfort they give, you don't have to roam here and there to get the medicines.
Those who have a lifestyle in which they just cannot afford to go to the physical stores can take
the benefit of ordering online medication.

 Simple procedure

People like to stay in their comfort and they can do it from the online mode without leaving their
home. There will no long queues to deal with, no judgmental people around you while you are
buying things related to the Erectile dysfunction pills online.

 Privacy

It is really better to buy medicine with which you are not very comfortable. This option keeps
you away from the gazing eyes of the people around you. You can buy erectile dysfunction pills
without any issues of embarrassment.

 Growth in usage of the internet

The e-Pharmacy market is projected at 15.2 % during the forecast period 2019-2024. The growth
of the e-pharmacy market is attributed to a rise in the number of internet consumers, increased
access to web-based and online services, and rising implementation of e-prescriptions in
hospitals and other healthcare services.

12
Weakness

 Opening medical store is a challenging process. You need to research the market, make
financial arrangements, medical store loans, obtain permits, hire staff, and much more.
 Sale without prescription: It is a known fact, and has been observed by judiciary. and
bureaucrats too, that one just need to call a retail pharmacy, or send a driver and
medicines are delivered at home.
 Storage and Logistics: Medicines in India must be stored below 30*C and 70% relative
humidity, to retain full potency over the entire shelf life. This would require constant air-
conditioning of products. Unfortunately, majority of retail pharmacies are not air-
conditioned, putting stability of medicines at risk
 Track and trace: We all know that pharmaceutical distribution system in India is totally
porous.

Opportunity

 There are medical Representatives and retired employees, unemployed experienced

medical sales representatives who can be encouraged, licensed and funded by the Govt to
open pharmacies in Rural areas.
 I have come across B Pharma and M Pharma degree holder having very poor knowledge
of Pharmacology. A short course training can be given to experienced personnel from
pharma field and certified to open Pharmacies.
 Pharmacies and pharmacists do a noble job of providing medicine and health care,
services to population at large. The adoption of innovative technology models will make
the entire process more robust, reliable, transparent, and efficient.
 Facilitate regulation for transparent and secure drug distribution system which, results in
product traceability and reconciliation throughout the supply chain. in case of quality
issue or nation-wide recall.

13
Challenges

 Lack of trained pharmacists: It has been documented as well observed by the court that
in several states, such as UP, number of pharmacies far exceeds the number of registered
pharmacists.
 Fake certificates: large number of either fictitious and unqualified persons or one person
using his registration certificate in several shops, are looking after such shops but the
state government has not taken any effective preventive steps,
 Fear of toxic ingredients: The medication delivered may contain very toxic ingredients,
such as drywall or rat poison. The medication may cause unexpected and dangerous side
effects to customers
 Delivery delays: Prescriptions can sometimes take up to 14 days to be delivered. A
person may need to collect the first 2 weeks of their prescription from a local pharmacy.
 Lost medications: It is unusual, but occasionally, packages can go missing in the mail,
and re-shipping the prescription may take time.
 Quality issues: Several unexpired drugs were then found to be unsafe and were
consequently recalled.as well as an expired drug sold by the pharmacist
 Regular supply shortages. If someone needy comes into the medical shop and
pharmacists had told them I don’t have what they want They will think this place never
has the thing that they need
 Trust issue: Sometimes packaging changes, other times the tablet’s colour. People who
are not literate will get confused and not trust what we are giving because they will think
that this does not look the same as what we gave before. It’s the same medicine but it’s
hard to convince them because it looks totally different.

Chapter -5

EXPERIENCE, LEARNING AND CONCLUSION

23
EXPERIENCES
It was extremely fascinating and exhilarating to work on this study as I was waiting for an
opportunity to study about factors influencing traditional and online purchase of medicine among
the people into the light.

The study started with framing the title and objectives, which was consistent with framing
questions. It was clear that I had to do a mixture of quantitative and qualitative study to achieve
my objectives. Therefore, it was only possible for me to conduct an interview with the
questionnaire.
Information regarding online purchase is collected through google forms from 62 respondents
Once the introductory part of the paper was completed, I started with data collection.
After collecting the data many facts came to lime light such even the educated people are not
comfortable in purchasing the medicine from the online apps because of various reasons such as
lack of trust on online apps, privacy, fear about fake products etc.
People are much more comfortable in purchasing the medicine from the medical shops, due
interaction with the profession’s pharmacist, and it is a traditional practice of the people.

Learning
 People prefer to buy medicine from the conventional/medical shop rather than online
pharma apps because of various reasons such as fear of fake products, privacy, Outdated
or substitutes medicines etc.
 Most of the respondent in south Bangalore are aware about online pharma apps yet they
