AFFIDAVIT OF SWAPPING

We, PLT CRISANDRA B AGGABAO and PLT RANDY D BACCAY, both legal age,
member of the Philippine National Police after having sworn to in accordance with
law hereby depose and state;

That I, PLT CRISANDRA B AGGABAO is presently assigned at Roxas Police

Station, Isabela Police Provincial Office while PLT RANDY D BACCAY is presently
assigned at Tuao Police Station, Cagayan Police Provincial Office.

That we are willing to be transferred and swapped assignments with each other of
which schedules and places of assignments are as follows:
