AFFIDAVIT OF LOSS

I, STEPHANIE CLAIR G. VANO., of legal age, single, Filipino and

resident of Andagao, Kalibo, Aklan, Philippines, after being sworn to in
accordance with law, hereby depose and say, that:

1. I am an employee of BPI-PHILAM LIFE ASSURANCE CORP., and

was issued a company mobile phone under a GLOBE Telecom post-paid
account for my official use, described as follows:

2. Sometime on February 2, 2018, I rode an UBER taxi from Makati

City en route to NAIA Terminal 2, I noticed that my above-described cellular
phone was no longer in my bag after I entered NAIA terminal 2 it probably
slipped out my bag while I was riding the taxi and I failed to notice it because
the taxi was full. I tried calling my mobile phone but it was already turned off;

3. Despite diligent effort on my part to look for my above-described

mobile phone, the same could not be found;

4. For all legal intents and purposes, I consider the above described
mobile phone and SIM card to be lost and beyond recovery; and

5. I am executing this affidavit to attest to the loss of the above-

described mobile phone and SIM Card No. 0927-317-5780 and 0956-759-
1081, to have the same blocked by the National Telecommunications
Commission and for all other legal intents and purposes.

Further affiant sayeth naught.
