PERMISSION SLIP AND WAIVER FORM

I, _______________________________, grant permission for my

son/daughter, __________________________, to participate in the English Festival 2020-2021
Educational Field trip entitled: “Get ENG Loud: Leading the future educators in the way to Outreach
One’s Potential through within” which requires transportation to a location away from the University
site. This activity will take place under the guidance and direction of the Faculty and Officers of the
College of Education Department, Mindanao State University – Lanao del Norte Agricultural College.

A brief description of the tour follows:

 Location of event: Mindanao State University – Main, Marawi City

 Date and estimated time of arrival/departure:
Arrival : June 20, 2021, 2:00pm
Return : June 23, 2021, 5:00pm
 Mode of Transportation: School Bus Service of MSU-LNAC
 Board and Lodging: Marawi De Hotel

I, as a parent and/or legal guardian, remain legally responsible for any personal actions taken by the
above named participant.

In signing the Parental Consent and Waiver, I am not relying on any oral or written representation or
statements mde by the university.

I have read and understand the terms of this consent and waiver, and recognize that it’s binding upon
me.
