For Students Continuing to the Doctoral Program

This form is to be completed when a student admitted in a Master’s degree program is continuing in the same
program for a doctoral degree immediately upon graduating with the Master’s degree. Please complete and return to
the Graduate Studies Office in 665 Baldwin Hall. An additional step is required to complete the change to PhD. You
must complete an on-line application (no fee required). This step is only done if form is completely approved. Please
see an advisor in 665 Baldwin Hall for details.
