b) Confirms CDC to obtain and verify my requisite details of Sub / IAS Account which is in accordance with my IPO
Subscription details provided at the time of subscription.
c) Indemnify CDC, its directors, officers and employees who shall not be held responsible for and do not undertake any
liability for any loss (including loss of business) and/or any direct, indirect, incidental, special, exemplary or
consequential damages that may be incurred or caused due to transfer of securities from my/our IPO Facilitation
Account to my/our Sub-Account/ Investor Account using this Form.
d) Declare that CDC shall not be liable or to any other person whatsoever, for accepting and processing this request.
