This letter refers to the incident which you were involved at Re: delayed releasing of
performance bonus 2021, due to the inaccuracy of the list you have released as
Payroll Administrator either intentionally or by negligence.
It is evident that this occurrence did not only happened once but twice in this
period, which further creates not only cause of delay and hampered other
management tasks/transactions, but also undermines the integrity of
company documents, policies and transactions. These unwarranted delays,
inefficiencies, inadvertence and inaccuracies either intentionally or through
negligence in the payroll documents to which does not only entail
additional/material cost but also damage to the company as well.
Knowing fully that the incident is a serious violation in the Company Rules
and Regulations, RE: RULE I – SERIOUS MISCONDUCT OF WILLFULL
DISOBEDIENCE
Taking into consideration the facts stated above, you are hereby required to
explain in writing within five (5) days from receipt hereof as to why you
should not be meted with appropriate disciplinary penalty.
