Annexure B

This is the acceptable list of documents that can be accepted as a valid photo id proof &
address proof.

Photo Identity Documents for employees (Any one of the below)

1. Valid Passport, which is not expired.

2. Government photo ID card issued by any of the following
(A) Organizations/Institutions
(B) Central Government or any of its Ministries
(C) Statutory / Regulatory authorities
(D) State Govt. or any of its Ministries
(E) Public Sector Undertaking (established under GOI or State Govt)
(F) State Govt. of J&K (Id card issued by the State Govt. of J&K is acceptable if
They carry the connotation - 'Government of Jammu & Kashmir, Civil
Secretariat, Srinagar/Jammu')
(G) Bar council
(H) Govt. of India to Persons of Indian Origin [PIO card]
(I) Defense Dept / Ministry of defense for Defense personnel & their dependants
(J) Public Financial institutions
3. PAN card
4. Election card / Voters ID Card
5. Permanent Driving license, which is not expired.
6. Photo credit card where the applicant is a primary holder and the card is not expired.
7. Ration card issued by Government of Andhra Pradesh (with photo).
8. Social security card issued by Government of Goa.
9. Photo debit card of a nationalized / private sector bank.
10. College photo identity card for fresh graduates. Fresh graduates would mean up to 12
months from the date of graduation. This to be mandatorily supported by any one of the
below:
(A) Hall Ticket
(B) Mark sheet of the college.
(C) Bonafide certificate from college.
11. Passbook of nationalized banks containing photograph.
12. Banker’s verification. The document must contain the employee name, photo and
signature duly attested by the bank. The date of account opening with the other bank must
also be mentioned.
13. MAPIN card (issued by NSDL)
14. Arms License issued by the State/Central Govt of India (containing Photograph of the
applicant)
15. Pension payment card issued by State/Central Govt. of India with Photograph of the
applicant.
Address Proof Documents for employees (Any one of the below)

1. Passport which is not expired

2. Permanent Driving license which is not expired
3. Telephone bill of Public and approved Private operators in the State
(Not older than 3 months from the date of joining)
4. Electricity bill of Public and approved Private operators in the State
(Not older than 3 months from the date of joining)
5. Bank account statement of a PSU / Private Sector / Foreign bank
(Not older than 3 months from date joining)
6. Ration card
7. Voters ID /Election ID Card (if it has address)
8. Letter from the society (only registered societies) - on the letterhead, which carries the
society's Registration No. The letter should carry the applicant's name & full address as
well as the signatories' full name, designation (Secretary, President) and address.
9. Monthly outgoings bill from Registered Housing society - on the letterhead which
carries the society's Registration No. The letter should carry the applicant's name & full
address as well as the signatories' full name, designation (Secretary, President) and
address.
10. Title deeds of the property duly stamped and registered
11. Letter from any recognized public authority (in original)
12. Last paid Life insurance premium paid receipt not older than six months from the date
of account opening.
13. Credit card statement where customer is a primary holder. This needs to be supported
by photocopy of credit card. Credit card should not to be expired.
14. Letter from income tax authority communicating PAN number to the employee.
15. Allotment letter from company to the employee allotting residential premises where
the accommodation is provided by the company to the employee. This to be signed by the
authorized signatory of the company.
16. Gas connection usage statement for pipe gas connections only for the below
companies:
(A) Mahan agar Gas Ltd, Mumbai
(B) Indrapasth Gas Ltd, Delhi Consumer gas connection card / book OR latest
Pipe gas bill. Gas charges paid receipt not acceptable.
17. Latest Property tax bill / Property tax paid Receipt.
18. Mobile post paid bill of public / private operators in the State.
19. Domicile certificate with communication address issued by Municipal Corporation
20. Arms license issued by the State/Central Govt. of India authority which contains
photograph of applicant.
21. Photo Id card with photograph (issued by J&K & Goa State Govt.) (Id card issued by
the State Govt. of J&K is acceptable if they carry the connotation –‘Government of
Jammu & Kashmir, Civil Secretariat, and Srinagar/Jammu)
22. Photo Social Security Card issued by the Central/State govt./Union territories.
23. Pension payment card issued by State/Central Govt. of India with photograph of the
applicant.
24. IRIS scanned household Card with photograph (issued by Andhra Pradesh govt.)
