NO 4555321Y NK RAMGOPAL DURING TREATMENT OF HIMSELF IN CIVIL
HOSPITAL

INTRODUCTION
1 No. 4555321Y NK RAMGOPAL is posted to the 1198 DSC Platoon with Paschim sub–Area HQ
Bhopal on 1 January night he got chest pain on during duty hours so he got treatment in MH Bhopal
and after that from 4th of January to 17 February he is on leave. after reaching his hometown he
gone to MH Jabalpur on 5th of January for further check-up but doctor on duty told him he should
come on medical OPD on 7th on January. but on 7th of January morning, he again got chest pain and
he got admitted nearby civil hospital by locals. the doctor there gave the emergency care and did the
angiography and found one blockage in heart because of emergency situation the required
treatment Angioplasty has done and after 48 hours of observation he got discharged on 9 th of
January and on 9th of January he admitted to MH Jabalpur for 5 days the he got discharged on 14 th
January. and from that date he is in sick leave from MH Jabalpur

2. It is proposed that sanction may please be granted for reimbursement of expenditure incurred in
the treatment of NK RAMGOPAL of no 4555321Y

DETAILED JUSTIFICATION
3. No. 4555321Y NK RAMGOPAL is posted to the 1198 DSC Platoon with Paschim sub–Area HQ

brother residence on the way when he is coming from his relative home again got chest pain, he is
alone at that time so he stopped and ask for help and then a person stopped called his family and he
got admitted nearby civil hospital by a local and his son. the doctor there gave the emergency care
and did the angiography and found one blockage in heart because of emergency situation and the
delay could be life threating the required treatment Angioplasty has to be done on emergency basis
and after 48 hours of observation he got discharged on 9 th of January and on 9th of January he
admitted to MH Jabalpur for 5 days the he got discharged on 14 th January. and from that date he is in
sick leave from MH Jabalpur

4. Expenditure incurred in the treatment of NK Ram Gopal of no. 4555321Y is Rs 1,39,603 (one lakh
thirty-nine thousand six hundred three only)
