CONSENT FORM

This informed consent is for Grade Six Learners whom the researcher invited to
participate in the study entitled: “Academic Struggles Experienced by the Grade Six
Learners of Liceo del Verbo Divino, Inc., Tacloban City.”

Introduction and Purpose of the Study

Teaching in the new normal set up is challenging, however it is really

significant and timely for the educators to learn and practice pedagogies that would
surely addresses learner’s learning needs. Allowing educators, curriculum planner
and implementer, as well as the institution to know the learner’s learning struggles
are totally beneficial since it will be utilized and considered as basis in strategizing
approaches and methods in teaching.
Intervention

As a participant in this study, you will be interviewed anytime (using google form), if
your schedule permits. You may raise any concerns or clarifications concerning
this study. 
 
Selection of the Participants
In this matter, we are inviting you to be part of the study. We will discuss with you
the details of this study at the beginning of the process. All 20 learners are
qualified to participate in this study. 

Study Procedure
The researchers are the principal investigator. We will also be the ones to explain the
details to ensure a smooth flow during the interview (google form). Please feel free to
ask questions about the study.
 
1. First, you will be asked, how effective learning in the New Normal has been for
you?
2. Second, we will ask you the struggles you experienced based on the following:
a. Self-regulation
b. Technological literacy and competency Use of strategies
c. Student Isolation
d. Technological Sufficiency
e. Technological Complexity
3. Third, how did these struggles affect your learning?
4. Fourth, how did you cope up with these struggles?
5. Fifth, how helpful has your school been in offering you the instructional resources
in the New Normal?
6. Sixth, what is/are your suggestion/s in coping up with this/these unprecedented
struggles in time of global crisis?

Risk
Participating in this study entails a risk, maybe you will be uncomfortable in
answering the different questions through an interview (google form). The
researcher hopes that this will not happen. 
 
Benefits
Being a participant in this study would be a great help to address the need of the
struggling Grade Six Learners in the new normal. This is also beneficial to the
teachers, curriculum planner, and to the institution so that they will be aware on
how to handle various learning preferences of the learners.
 
Confidentiality
Your response to this study will be handled with confidentiality. The researchers
will give assurance that there will be assigning codes instead of having your name.
Also, after the interview, the record and the transcription will be kept in a safe area
where the researchers will only be the ones to have access to it.

Voluntary Participation
Your participation in this study is voluntary. It is up to you if want to participate or
not. If you decide to be part of this study, you are going to sign a consent form.
You are free anytime to withdraw your decision.
