INFORMED PARENTAL CONSENT FORM

I, Mr. John Cliford Alvero, a faculty and at the same student at San Pablo Colleges invite your
child to take part in my research study “INSTRUCTIONAL MANAGEMENT PLAN FOR
DEVELOPING GLOBAL CITIZENSHIP COMPETENCIES OF SENIOR HIGH
SCHOOL LEARNERS.” The study, as well as your rights as a participant, are described
below.

Description: This study will examine your child’s perception with regards to the extent of
instructional practices manifested by his/her Social Studies teacher/s. Your child will be given a
survey form that will be answered with the use of internet. Your child’s identity will not be
revealed to anyone but the principal investigator and his designated research associates.

Confidentiality: Children’s answers will be not be associated with their name. Rather, each
child will be given an identification number on the interviewer’s sheet.

I agree to the researchers using my child’s real name in this research and any
publications the results from the research.

Freedom to Withdraw or Refuse Participation: I understand that my child has the right to
stop answering the survey form at any time, or to refuse to answer any of the interviewer’s
questions without prejudice from the investigator.

My name is John Cliford Alvero. I’m a Senior High School faculty and at the same time Master
of Arts in Educational Management student at San Pablo Colleges, San Pablo City. I am inviting
you to participate in a research study “INSTRUCTIONAL MANAGEMENT PLAN FOR
DEVELOPING GLOBAL CITIZENSHIP COMPETENCIES OF SENIOR HIGH SCHOOL
LEARNERS.” Your parent(s) know we are talking with you about the study. This form will tell
you about the study to help you decide whether or not you want to take part in it.

What is the key information about this research study?


The following is a short summary of this study to help you decide whether you want to be a part
of this study. Information that is more detailed is listed later on in this form.

The purpose of this study is to assess the extent of instructional practices manifested by your
Social Studies teacher/s. You will be asked to answer the Google form that will be sent to you by
the researcher. I expect that you will be cooperating in this research study. The results of this
study will benefit the learners with regards to the instructional practices in teaching Social
Studies.

Why is this study being done?


The purpose of the study is to develop an instructional Management Plan (IMP) that will later on
be used as a guide by this institution in producing global citizens.

What do I need to do?


If you decide to be in the study, I will ask you to answer the Google form that contains 60 Likert
scale - item statements.

What are the benefits to me?


Taking part in this study may not have direct benefits to you, but it will help this institution in
producing graduates that can answer the demand for opportunities to engage in relevant,
meaningful activities that enhance students' global perspectives and help them to contribute to a
more peaceful, environmentally secure and just world.

Are there any risks to me if I decide to be involved in this study?


There are no foreseeable risks because you answers will be not be associated with your name.
Rather, each child will be given an identification number on the interviewer’s sheet.

How will my information be protected?


Your responses will be treated with utmost confidentiality. The results of this study may be used
in reports, presentations, or publications but your name will not be used. Additionally, the data
will be stored in a password protected computer), only the researcher and his designated research
associates will have an access to the data. The data will be retained up to 3 years after the IMP is
completed.

Do I have to be in the study?


No, you don’t. The choice is yours. Your participation in this study is completely voluntary. No
one will get angry or upset if you don’t want to do this. And you can change your mind anytime
if you decide you don’t want to be in the study anymore. Refusal on this study will not affect
your grade.

What if I have questions?


If you have questions about the study, you can ask me now or anytime during the study. You can
also call me via messenger. You will receive a copy of this form for your records.
