AFFIDAVIT OF COMPLIANCE FOR POST KIDNEY TRANSPLANT MEDICATIONS

I, CATHERINE DELA PAZ TORRES, 55 Years Old, Filipino citizen, Widow with
Residence and postal address at #002 5th Street Pacheco Subdivision, Dalandanan, Valenzuela
City, having been sworn in accordance with law, hereby depose and say:

1. That I just getting help from her immediate family and relatives and she wants to live
a normal life.

2. ROSARIO SAULAN, Retired Government Employee and expected an amount to be

receive on a monthly basis

A. That my son ALFREDO TORRES, is willing to support me in my post kidney

transplant medication expenses through his SALES AGENT with a share of Five
Thousand Pesos (P 5,000.00).

B. That my aunt ROSARIO SAULAN who is a Retired Government Employee is

willing to give Ten Thousand Pesos (P 10,000.00) monthly

3.That I can comply with my post kidney transplant medication for the next three years
given the above mentioned support.

4. That I am willing to seek assistance from different agencies of the government in case
these support will not suffice.

5. That I execute this affidavit to attest to the truth of the foregoing and for whatever legal
intents and purpose this may serve.
