I.

INTRODUCTION
Community health nursing is a nursing practice focusing on the healthcare of individuals,
families and groups with a community. It’s main focus is to improve the quality of life and health of a
population by preserving, protecting, promoting, or maintaining health. Instead of focusing on
institutional care, community-based nursing brings healthcare within the reach of the community.
Factors influencing health services such as political process affecting public health nursing.
Community-based nursing focuses on the fulfillment of the healthcare needs of an individual or
family.

II. INITIAL DATA BASE


A. Family Structure, Characteristics and Dynamics
1. Members of the household and relationship to the head of the family
The family is composed of 2 members, the mother and her only son.

2. Place of residence of each member


- They are both currently living in M Roxas, BC Unit 1, Trancoville

3. Type of family structure


-Nuclear

4. Dominant family members in terms of decision making


- According the mother, his son is the one responsible for decision making

5. General family relationship/dynamics


- The son is responsible in terms of decision making, budgeting and working for living.
The son is more mature and does his obligations as a son.

B. Socio-economic and cultural characteristics


1. Income and Expenses
- According to the son he is the only one working and his income is ranging from 5000-
10,000 pesos every month.

2. Educational attainment of each family member


Mother - Highschool
Son - College Graduate

3. Ethnic background and religious affiliation


- According to the mother they are Roman Catholic but not active in church.

4. Family traditions, events or practices affecting members health or family functioning


-None

5. Significant others
- According to the mother they don’t have significant others

6. Relationship of the family to the larger community


- According to the son they are not member of any organizations in the community.

C. Home and Environment


1. Housing
- According to the son, the house that they are currently living is rented. It is small and
exact for just two persons. It is made up of wood and consists of a double deck bed with
a small window.

2. Kind of neighborhood
- The neighborhood are also a security guards of an apartment. They are very helpful and
generous especially to the family because they know their situation.

3. Social and health facilities available


- According to the son there a re no social and health facilities available.
4. Communication and transportation facilities available
- According to the son there are jeepneys available nearby.

D. Health status of each member


1. Current and past significant health condition/s or illness/es or Beliefs and practices
conducive to health and illness
- According to the son, her mother is very addicted before in smoking. And currently her mother
is diagnosed with Lung Cancer

2. Nutritional status
- According to my observation, the mother and son are underweight. Prior to my
interview, they have inadequate intake of food. There are days that they do not eat
because of their financial status.

3. Developmental status

4. Risk Factor assessment indicating presence of major contributing modifiable risk
factors
- According to my assessment the mother needs an immediate interventions and actions
due to her present illness

5. Physical assessment
- According to my observation the mother needs a thorough check up especially with her
illness. On the other hand, the son seems to be weak and inactive.

6. Results of laboratory/diagnostic and other screening procedures supportive of


assessment findings
- According to the medical practitioner the mother needs to be admitted because she is
already in a Stage 4 Lung Cancer and needs a chemotherapy.

E. Values and practices on health promotion/maintenance and disease prevention


1. Immunization status
- The Immunization status is complete from the mother to son.

2. Health lifestyle practices-specifiy


- The family’s lifestyle is not healthy. With their financial problems, sometimes they do
not eat within a day. They also tend to eat in can foods which is not good for an everyday
meal because may contain dangerous bacteria which can cause serious illness, they also
tend to eat instant noodles which lacks important nutrients such as vitamin A, vitamin C,
B 12 and more.

3. Adequacy of
a. rest/sleep
- The mother gets adequate rest and sleep due because she only stays at home while
his son doesn’t get enough adequate rest/sleeo due to his shift as a security guard of
the apartment.

b. exercise/ activities
- The mother doesn’t exercise much and prefers to lie down or sit at home while his
son do exercise sometimes by walking and cleaning the garage.

c. Use of protective measures


- The son doesn’t have a bulletproof vest during his duty.

d. Relaxation and other stress management activities


- The son socialize with the tenants of the apartment and this helps him to unwind his
problems.
e. Opportunities with enhance feelings of self worth, self efficacy and sense of
connectedness to self
- The tenants of the apartment always talks with the son and reminds him that they
are there for him if he needs help regarding with the situation of his mother and
according to my observation this helps him mentally and emotionally.

f. Stress management or other healthy lifestyle practices


- According to the son, he loves to sweep the garage even though it is not his job to
do in the building because it helps him calm his mind.

g. Use of promotive-preventive health services


- According to the neighbors of the family that I have interviewed, the tenant from
the Unit 40 offered the son to admit his Mother in the hospital prior to the condition
of his mother which is already in stage 4 Lung Cancer but the the only problem is
who will be the guardian of her mother because the son’s duty shifts is 24 hours.
