General Activities for Surrendered, Abandoned, and Involuntarily Committed Children:

If the child was conceived out of the birthm other's relationship with
another m an, by law, the child is considered a legitim ate child and shall
bear the sum am es of the legal husband and the birthm other. The deed of
voluntary com m itm ent m ust be signed by the m arried parents (Refer to
Fam ily Code, see also attached Board Resolution No. 17-004).

11.2.Birth certificate of the parents

7. Notify the residential facility in the event that a person
comes to the hospital to claim the child.

claiming the child to prove filiation.
