CONSULTATION REPORT
HISTORY OF PRESENT ILLNESS: This patient was referred to me by Doctor Donnie Dukes for
evaluation and recommendations regarding the patient rotation.
Mr. Sign was a 62-year-old gentleman who presented with complaints of validation.
On his face.
He states that he had tried topical gel parenthesis, metro gel 1%) but have not believed this has
alleviated his rosacea.
He states he had laser treatments for his rotations three times at the university and two times at Metro
Hospital, most recently six months ago, and several other instances at other institutions he presents
today for evaluation for further rosacea treatment.
At the university. He has a history of skin cancer in his status post. Excision of basal cell carcinoma. Uh,
of the right upper extremity. Nine months ago. She also has a history of tinnitus, hypothyroidism,
cardiovascular disease status, post 4 vessel cabbage two years ago. Status post.
India pasty status post angiogram history of seizures, history of high cholesterol, history of hearing loss
tinnitus status post penile implant Times 3 status post back surgery times to the most recent Women
Act in the last year. Stable system selecting as a child instead of false left ankle surgeries.
He has no history of diabetes or hypertension
SOCIAL HISTORY: He is not married. He does not smoke, drink alcohol, or use illicit drug

MEDICATIONS: Synthroid .145 milligrams daily crafter, 10 milligrams daily Tegretol 100 milligram
pub ID aspirin. 81 milligrams daily. And Metro gel, which was stopped a few weeks ago one.
The patient has St areas of rosacea along the left nasal tip left medial cheek and right cheek along the
angle of the mandible.
The failures are not significantly inflamed, nor are they a parent, except on close inspection.
He does have evidence of lower little blepharoplasty with rounding of his lateral candy.
Mr. Signs has very welcome for authorization, not a significant cosmetic defect at this point, so we
recommended that he continue with the metro gel 1% topical.
We discussed with him that he will investigate further treatments and let him know if we have further
treatment of this and discuss his case.
