WE, DENNIS DIOCOS and HARLENE DIOCOS, both Filipinos, of legal ages, married and residents
of Tabanlo, Mariano, after having been duly sworn to in accordance with law do hereby deposed and

1. We have known Lycon Mendras and Nichole Mendras as we are neighbors for a long period
of time;

2. Lycon Mendras and Nichole Mendras have been living together as married couple. They
were married on October 14, 2000 at Tabanlo, Mariano;

3. The parent’s name of LYCON MENDRAS in the marriage certificate recorded on the Local
Civil Registrar and on the National Statistics Office are LYKA BADIAN AND PELAYO
MANLAKAT ;

4. That the correct parent’s name of LYCON MENDRAS that should appear in the record Local
Civil Registrar and on the National Statistics Office LEONILA ROSAL AND FELICIANO
MENDRAS;

5. The parent’s name of NICHOLE MENDRAS in the marriage certificate recorded on the Local
Civil Registrar and on the National Statistics Office are LETICIA ROMAN AND FELICIANO
MENDRAS ;

6. That the correct parent’s name of NICHOLE MENDRAS that should appear in the record
Local Civil Registrar and on the National Statistics Office MARIA PUNZALAN AND ALLAN
BACAT;

7. LYCON MENDRAS and NICHOLE MENDRAS are processing the application for the correction
in the entry of the of Marriage Certificate of LYCON MENDRAS and NICHOLE MENDRAS at
the Office of Civil Registrar;

8. We attest to the truth and veracity of the foregoing facts and circumstances. That we
execute this affidavit to support the application of LYCON MENDRAS and NICHOLE
MENDRAS for Correction in their Marriage Certificate at the Office of the Civil Registrar and
for whatever legal purposes this may best serve.
