AFFIDAVIT OF LOSS

I, SEON HO, Filipino, of legal age, married, and a resident of Makati City, after being duly sworn to
in accordance with law, hereby depose and state:

1. That, I am a holder of a driver’s license;

2. That, my driver’s license which I safely kept and which was always in my possession was lost;

3. That, despite due and diligent search and efforts made, the said driver’s license could no longer be
located;

4. That, as such, I now believe that my driver’s license is lost beyond recovery; and

5. That, as such, I am executing this Affidavit of Loss in order to attest to the truth of the foregoing,
and for whatever purposes this may serve.

IN WITNESS WHEREOF, I have hereunto set my hand this 26 January 2022 at Makati City,
Philippines.
