FOR APPLICANTS

1. Please read the Entry Guidelines and Conditions carefully before and after completing the
entry form. Ensure that all sections (A to G) are complete.

2. Your entry form should be computer generated or typed; hand written applications will not be
accepted, however, the writing must be clear and legible.

3. Please label your DVDS/FILES with the following details: Film Title, Entrant’s Name.

4. EMAIL: Please email one copy of the completed entry form as attachment (.doc word file, pdf)
to adelaideundergroundfilm@gmail.com. (You may leave out payment details when sending via
email)
