AFFIDAVIT

I, MA. LORGIE DE PAZ ELORDE, of legal age, Filipino, single and a resident of Brgy.
104, Salvacion, Tacloban City after having been duly sworn to in accordance with law, hereby
depose and say:

1. That I am the sister of LORJILYN DE PAZ ELORDE who was born on 25 July 1988
in Tacloban City;

2. That she was baptized at Sto. Nino Parish, Tacloban City on 24 December 1988;

3. That there was a clerical error in her Certificate of Baptism especially, the name
extension of her father, appearing therein as “JOSE ELORDE” instead of “JOSE
ELORDE JR.”;

4. That the name extension of her father in all of her pertinent documents such as her
Certificate of Live Birth and her school records is appearing therein as “JOSE ELORDE
JR.”;

5. That I am categorically stating that “JOSE ELORDE” and “JOSE ELORDE JR.” also
refers to one and the same person;

6. That this affidavit is not contrary to law, morals, public policy and have no intention of
fraud;

7. That finally I am executing this affidavit to attest to the truth and veracity of the
foregoing facts as well as for all lawful purposes the same may serve.
