AFFIDAVIT of COHABITATION
We, MARISA SELANOVA TAGALO and PATERNO HAMTING TULAYTAY, JR., both
of legal age, Filipinos and residents of Brgy. 66, Paseo De Legaspi, Tacloban City, Philippines, after
having been sworn to in accordance with law, do hereby depose and say:

1. That we are the beneficiaries of the Yolanda Permanent Housing project at Block 29,
Lot 22, Knightsridge Residence, Brgy. Camansihay, Tacloban City;
2. That we have been living together as husband and wife without the benefit of marriage
continuously and without any interruption for over eleven (11) years now;

3. That our common-law partnership is known to the community in Brgy. 66, Paseo De
Legaspi, Tacloban City;

4. That this affidavit is not contrary to law, moral, public policy or order and with no intention
to defraud anyone;

5. That we are executing this affidavit to attest the forgoing facts and for whatever purpose the
same may serve;
