AFFIDAVIT

I, ROSALINA D. INOJALES, Filipino, of legal age, married and presently

residing at Panas, Candijay, Bohol, Philippines, having been duly sworn in accordance
with law, hereby deposes and states that:

1. That I was a Barangay Kagawad of Barangay Panas, Candijay, having served from
December 1, 2007 until June 30, 2018, and thus has served as such for three
complete term;

2. That I was not employed in the government during my term and that I did not
receive any form of salary or compensation from such, except honorarium for
holding a position in an ex-officio capacity, during my term of office;

3. That I execute this affidavit to be able to attest to the truth of the foregoing facts,
to support my application for grant of Barangay Official Eligibility, and for
whatever legal purpose this may serve best.

IN WITNESS WHEREOF, I have hereunto set my hand this 6th day of November,
