AFFIDAVIT OF LOSS

I, JOAN ESTELLA RADAZA, of legal age, Filipino, married, and a

resident of Brgy. Lapu-lapu, Butuan City, under oath, depose and state:

1. That I am a holder/owner of a Transcript of Records, issued by Father

Saturnino Urios University, Butuan City;

2. That I recently discovered that the said Transcript of Records is now

irretrievably lost and can no longer be found despite diligent efforts to
locate the same;

3. That I am executing this affidavit to attest to the veracity of the

foregoing and for whatever legal purpose it may serve.
