JOINT AFFIDAVIT OF COHABITATION

We, _________ PADIL and _____________ DEL ROSARIO, both

of legal ages, Filipino, and residents of Brgy. Tacloban City, after being
sworn to in accordance with law, depose and state that:
1. That we have been living together for more than ten (10) years
but without the blessing of marriage;

2. That during that period, we have been living together at Brgy.

78 Fatima Village, Tacloban City;

3. That there are no existing legal impediments in our relationship

to marry each other during such period of our cohabitation;

4. That we are executing this affidavit to attest to the truth of the

foregoing statements and for whatever legal purpose this
Affidavit may serve.
