The Class is divided into 3 groups.

Each group will be doing the the questions in the box


intended for their group.
The group should have one output after their group discussion. It will be presented in
powerpoint. 
You should select a leader and assistant leader. The groupings are shown here in
LMS.  The leader will be the one to submit the output together with the documentation
of your discussion and collaboration with narrative.
 
Group 1:

1. A nurse is conducting an initial comprehensive assessment of a 25-year-old client who has been
admitted to the health care facility with severe pain in the abdomen. During the assessment, the client
informs the nurse of being treated for a peptic ulcer. The client tells the nurse of having migraines and
taking aspirin to control them. The client is afraid of having taken too much aspirin. The client appears
pale, and, on examination, the nurse notes that the pain is originating around the stomach area.
a. How should the nurse categorize the information provided by the client into subjective and
objective data?
The nurse can categorize the information by interviewing the patient which is the subjective data and
observing and doing physical examination which is the objective data.

2. A 65-year-old client with clinical depression is being assessed by an RN. The client is reserved and
withdrawn. The health care staff caring for the client has reported lack of communication and angry
outbursts.
