Each team is tasked to contact the institutions assigned to them for updates. 4. A notification/ letter on findings shall be sent to the applicant school on or before December 9, 2019 once the team is finished with the document evaluation. ‘The schedule for validation visit shall be indicated in the letter. 5. The schedule of validation visit/ ocular inspection shall be from December 9, 2019 to January 15, 2020. The templates for the reports are in the enclosures. 6. The deadline for the submission of narrative reports and accomplished QAD Forms 5A and 5B is on February 14, 2020. 7. Participation of teachers is subject to the no-disruption-of-classes policy. It is highlighted that all teams secure a Certificate of Appearance from the applicant- schools during the validation visit/ ocular inspection. 8.
