WAIVER FOR RETURNING RESIDENTS

That I am a resident of the Kingdom of Bahrain who is currently on visit in the Philippines;

That I am fully aware of the security situation in Bahrain, and the Philippines Alert Level 2
security classification of the prevailing situation therein ;

That I know the consequences of the possible escalation in Bahrain on my safety and security,
and that of my family;

That I am willingly and voluntarily undertake travel to the Kingdom with my wife,
______________, with Philippine Passport No. ______;

That I take full responsibility for my decision and release the Philippine government and its
instrumentalities, that any blame that might arise in relation to my decision to travel to and stay
to Bahrain;

That I will not hold the Philippine government accountable for my safety and security in
Bahrain.
