AFFIDAVIT OF LOSS

I, ELDON JUN MAAGAD ABALLE, Filipino, 27 years of age, single and a resident
Iponan, Cagayan de Oro City, after having been duly sworn to an oath in accordance with
law, do hereby depose and say that:

I am the lawful holder and owner of the following item/s:

a. Driver’s License (PROFESSIONAL License) issued by LTO – Cagayan
de Oro City;

The said item was in the possession until I found that the same is missing;

Despite diligent and earnest efforts to locate it, the same proved futile;

The above-mentioned item/s is/are now lost and beyond recovery;

My Driver’s License has not been confiscated by any law enforcement agency and I
have no violation or pending or unresolved case for apprehension, alarm, suspension or
revocation;

That I am executing this affidavit to attest the veracity of the foregoing facts and
circumstances and for whatever legal purpose this may serve.
