AFFIDAVIT

I, JOJO PARDO ACOBA, of legal age, Filipino citizen and resident of Brgy.
#46 Nalbo, Laoag City, Ilocos Norte, after being duly sworn to an oath in accordance
with law do hereby depose and say;

1. That I was issued a company Identification Card from NobleWork Staff, Inc.;

2. That the above-mentioned ID were issued by the respective agency;

3. The said Identification Card is no longer in my possession;

4. That sometime July, 2020 said ID was lost while I’m driving a motorcycle;

5. I exerted thorough effort and looking for such Identification Card but to no
avail, hence I now consider the same to be lost and beyond recovery;

6. Further, I am attesting that said Identification Card were not confiscated nor I
was apprehended or caught by any authority;

7. I am therefore executing this affidavit to attest to the truth of the foregoing, as

well as for all legal purposes this may be worth.
