Vintar, Ilocos Norte, Philippines, under oath and states:

1. We are the Barangay officials, respectively of Brgy. Malampa, Vintar,

Ilocos Norte, Philippines, Philippines;

2. This affidavit is executed to clarify information about ALLAN BUTAC

GALAS and ANGELIE MARIE CALAUTIT;

3. That ALLAN BUTAC GALAS and ANGELIE MARIE CALAUTIT is our

constituent in the Barangay;

4. That ALLAN BUTAC GALAS and ANGELIE MARIE CALAUTIT

have been a relationship since 2014 and living together as husband and
wife under the same roof, continuously and without any interruption,
since 2017 or a period of more than five (5) years“;

5. That during their cohabitation they are born ANICA YZABELLE C.

GALAS;

6. We know the foregoing facts because we live in the same neighborhood or

community and we are the friends and close acquaintance; and
