We, BIENVENIDO VILLANUEVA ALEGA and FLORENDA A.
QUIDILLA, both of legal ages, Filipino citizens and residents of Brgy.
Paratong, Paoay, Ilocos Norte, Philippines, under oath and states that:
1. We are law abiding citizens and a residents of the Republic of the
Philippines;
2. This affidavit is executed in behalf of RENATO AGATOL;
3. The above-named person is our neighbor since birth;
4. Renato Agatol is the son of Isaias Agatol and Celedonia Quidilla
Agatol of Brgy Paratong, Paoay, Ilocos Norte, Philippines;
5. We know all the foregoing facts because we are long time
acquaintances and have witnessed how Renato grew up and
raised his family;
6. This affidavit is being made by us for the purpose of assuring that
Renato Agatol is the son of Isaias C. Agatol and Celedonia Q.
Agatol who married Miolita Almadin, a lass from Sto Tomas,
Davao del Norte;
7. Further, this is being made for the purpose of claiming the
benefits from the insurance of Miolita Almadin Agatol being
Renato Agatol, the legal spouse and one the beneficiaries; and
8. We are executing this affidavit to attest to the truth hereof and
for whatever legal purposes this may be worth.
