AFFIDAVIT OF TWO (2) DISINTERESTED PERSONS

We, LEONIDA L. MACAWILE and KAY LOUIELLA S. REPULDA, both of
legal age, Filipinos, married and long-time residents of Brgy. 76, Fatima Village
and B4 Lot 6 P4-Ext. V&G Subd.,, Tacloban City respectively, after having been
sworn to in accordance with law, hereby depose and state: THAT--1. We personally know the Spouses JESUS M. SIOZON, JR. and
TERESITA PONFERRADA SIOZON, residents of 110 Gurami St., B4 L54 P4
V&G Subd., Tacloban City and long time family friends;
2. We learned that their second born child, a BABY BOY, was born on
November 8, 1985 in Tacloban City, and was thereafter baptized in St. Jude
Parish Church on December 25, 1985 as JESUS P. SIOZON III.
2. We also learned that due to the negligence in the registration of the
name with the Office of the Civil Registrar of Tacloban City, the numeral III was
omitted, hence what appeared then and now in the certificate of live birth was
only JESUS P. SIOZON.
3. We are not related by blood or affinity with the Spouses JESUS M.
SIOZON JR. and TERESITA P. SIOZON, but we used to be good old
contemporaries during their lifetime;
4. We personally vouch for the truth about the fact of birth of JESUS P.
SIOZON, also known as Jesus P. Siozon III, being one and the same person, to
aforenamed spouses on November 8, 1985 in Tacloban City;
5. This affidavit is executed to prove the foregoing facts as well as for
whatever legal purpose this may serve.
IN WITNESS WHEREOF, we have hereunto set our hands this 6 June
