JOINT AFFIDAVIT RE:

FACT OF BAPTISM & IDENTITY
We, TERESITA LORA ESTRADA and VIOLETA VERGARA, both of
legal age, Filipinos, and residents of Manila and Tacloban City respectively,
after having been first sworn according to law, hereby depose and state:
THAT--1. We personally know CATHERINE LORA ESTRADA, who was born on
August 3, 1979 in Tacloban City, Leyte to herein first affiant TERESITA LORA
ESTRADA and late spouse DANILO B. ESTRADA. A copy of her Certificate of
Live Birth and Marriage Contract of Parents are hereto attached.
2. We personally know that she was baptized as CATHERINE EUNICE
LORA ESTRADA in Brgy. Salvacion Chapel, Parish of Transfiguration, Palo,
Leyte on September 8, 1999, as shown in her Certificate of Baptism issued
by the Fr. Isidoro Villarino hereto attached. As a matter of fact, herein affiant
VIOLETA VERGARA is one of the godparents of Catherine Lora Estrada.
3. However, the fact of baptism was not transmitted to the main Parish
of Transfiguracion due to inadvertence, hence no record of the said baptism
was recorded in the parish church of Transfiguracion, Palo, Leyte.
4. Moreover, based on her Certificate of Live Birth, Catherine Eunice
has been well known in the community as plain CATHERINE LORA ESTRADA,
and that she has used the same name in her school records and in other
public documents.
5. We declare that the name CATHERINE EUNICE LORA ESTRADA in the
hereto attached Certificate of Baptism and CATHERINE LORA ESTRADA as
appearing in her Certificate of Live Birth as well as other public documents
refer to one and the same person.
6. We further pray for the recording of the fact of baptism of
CATHERINE LORA ESTRADA in the records of the Transfiguracion Parish
Church, Palo, Leyte
IN TRUTH WHEREOF, we have hereunto executed this affidavit to
attest to the truth of the foregoing facts for whatever legal purpose, this 25
August 2005 at Tacloban City, Philippines.
