JOINT AFFIDAVIT

WE, ANITA L. MERCENES, widow, and DOMINGO R. OTIC, JR., widower,

both of legal ages, Filipinos, and residents of Brgy. 38, Sabang District, Tacloban City,
under oath declare as follows:

1. We know personally Alfredo Piaduche Zamora and Fidela Romero Kho,

both residents of Imelda Avenue, Tacloban City, who lived as husband and wife and
represented themselves as such.

2. We likewise personally know Ronelo Jose Kho Zamora, who is now

thirty-six (36) years and eight (8) months old, married, and a resident of Imelda Avenue,
Tacloban City.

3. Ronelo Jose Kho Zamora is the child of Alfredo Piaduche Zamora and
Fidela Romero Kho, having been born at the Bethany Hospital, Tacloban City on August
9, 1970.

4. We actually know that Alfredo P. Zamora and Fidela R. Kho were not
legally married but in the Certificate of Live Birth of Ronelo Jose Zamora, it appears that
the parents of Ronelo were married on October 11, 1969; this was entered as such
because Fidela R. Kho was only 16 years old when she gave birth and was confused and
afraid of her parents.

5. Ronelo Jose Kho Zamora is one and the same person who is also known as
Ronelo Jose R. Zamora born on August 9, 1970 at the Bethany Hospital, Tacloban City.

6. We are executing this affidavit to attest to the truth of the facts above-
stated and to cause the late registration of the birth of Ronelo Jose R. Zamora as his birth
was not duly registered with the Office of the Local Civil Registrar of Tacloban City.
