MANGILAYA, Filipino citizen, of legal age, and with residence address at Union, Libertad, Antique, and I Gretchen Barraca, Filipino citizen, of legal age, and with residence at Solido, Nabas, Aklan after having been duly sworn in accordance with law, hereby depose and state: 1. That we know RALPH TOLENTINO and ANALIE A. TOLENTINO as the parents of the child RHODEN A. TOLENTINO, and have known both since JUNE 24 2008. 2. That we also know their child RHODEN A. TOLENTINO and have known her since DECEMBER 19, 2010. 3. That we are not related by consanguinity or affinity to the child's parents. 4. That we are executing this Affidavit to attest to the truth and veracity of the foregoing facts and in compliance with law. FURTHER AFFIANT SAYETH NONE.
