 Alternate offers are not accepted.

 Items should be approved by FDA or the CE mark.
 The local agent should be registered in the Medical Supplies Administration.
 If a device or equipment needs reagents, solutions, controls, consumables, options and/or
disposables to perform a complete test, please quote all items necessary in order to perform
a complete test. Quantities for these quoted items (reagents, solutions, controls,
consumables and/or disposables) should also be quoted for 3 years, and have a fixed price
for 6 years.
 The offer should clearly state that there are no additional calibrators, solutions, controls,
and/or consumables other than those quoted. Any necessary items to perform the test that
have not been quoted shall be provided by the supplier free of charge.
 Quote maintenance for 3 years after 3 year warranty period.
 Mention the number of tests/packs and the shelf life for reagents, solutions, controls,
consumables and/or disposables to perform a complete test, as well as their shelf life if
applicable.
 Including catalogues for all items.
 Local training should be provided for end user.
 All items offered must be priced in Kuwaiti Dinars.
 If sample is required please provide it in its original packaging.
 Abide with all conditions of the tender as stated in Arabic in the tender booklet.
 Please offer the newest model available from the manufacturer unless otherwise stated.
