JOHANNES Y. DORADO, LGOO VII, Cluster Head of the Department of Interior and Local Government –Leyte,
as Speaker/Lecturer during the Lecture-Seminar on the Conduct of Parliamentary Procedure and the crafting of the
Internal House Rules and Procedures of the 15 th Sangguniang Bayan of the Municipality of Tanauan, Leyte on July 30
to August 1, 2019 at Sabin Resort Hotel, Ormoc City,.

Witnesseth that:
LGU- TANAUAN, LEYTE
Whereas, ______________________________________ is contracting out JOHANNES Y. DORADO as
Speaker/Lecturer on the said event, with an honorarium of TEN THOUSAND PESOS ONLY (PHP10,000.00).

Whereas JOHANNES Y. DORADO agrees to undertake his duties and responsibilities based on the deadline
set and agreed upon.

Now, thereof for and in consideration of the foregoing premises, the parties agree as follows;

 THAT HE WILL PERFORM AS A LECTURER/SPEAKER ON JULY 30-AUGUST 1, 2019.

 THAT HE WILL RECEIVE HIS HONORARIUM/FEES AFTER HE WILL COMPLETELY FINISHED
HIS JOB.

In witness whereof, the parties have hereunto and these presents affix their signatures this ____ day of
