The nature and cause of the charges and rights under the law were fully explained
to the respondent in Tagalog. He opted not to ask for preliminary investigation nor
execute a waiver of Article 125 of the Revised Penal Code. He moved that the
case/s be submitted for appropriate resolution by the Office of the City Prosecutor.
Furthermore, respondent was informed that the inquest proceeding with the OCP-
Pasig City will be recorded. Thus, he is giving his consent thereto.

mga karapatan ko sa ilalim ng batas. Pinipili ko na huwag nang magpaimbestiga
o sumailalim sa preliminary investigation. Nais ko i-sumite ang nasabing kaso sa
Office of the City Prosecutor para sa nararapat na kapasyahan. Naipalam din sa
akin na i-rerecord ang paghaharap sa OCP-Pasig City at binibigay ko ang aking
pahintulot sa nasabing pag-record.
