JOINT AFFIDAVIT

WE, THE UNDERSIGNED POLICE OFFICERS, both of legal age, bona fide members of
Philippine National Police presently assigned at intelligence section of Brgy, San Pedro, Puerto Princesa
City Police Station, after having been duly sworn to in accordance with law, do hereby depose and say:

1. That upon intelligence information from our informant there will be a delivery of
Methamphetamine Hydrochloride in the house of a certain John Doe which will be delivered by a
delivery van with plate number: 21222.

2. That on March 3, 2022 with in the vicinity of the house of John Doe, conducted surveillance
operation regarding a suspected delivery of undetermined amount of Methamphetamine
Hydrochloride.

3. That on or about 4;00pm of March 3, 2022, a delivery van with plate number: 21222 arrived and
delivered to the house of John Doe several boxes of suspected Methamphetamine Hydrochloride.

AFFIANTS FURTHER SAYETH NOT.
