SOAP NOTE
DATE OF ATTENTION: 28-March-2012
SUBJECTIVE: Polytraumatized male patient is brought by the emergency room.

OBJECTIVE: Repair the multiple fractures that it has
Take full body X-rays and CT scans to rule out possible brain and internal injuries
ANALYSIS: He had a fall from some stairs of approximately 21 meters in the Sistine
Chapel, having multiple fractures in the left clavicle, right ulna, lumbar spine, left
proximal phalanges, right tibia and fibula, left femur, left knee joint.
It is very possible that in the humerus and clavicle the brachial and axillary arteries are
affected, on the part of the proximal phalanges the dorsal digital arteries are affected
And on the part of the lumbar vertebrae, the abdominal aorta must be taken into account,
since it may also be affected.
The patient was asked to have a CT scan since he may have a neurovascular injury due to
the strong trauma he suffered.
PLAN: For the treatment of all the fractures that the patient has, it will be immobilization
with a splint, open reduction and internal fixation using metal rods, screws or plates to
repair the bone and keep it fixed under the skin.
For the left clavicle, intramedullary nails will be used.
Plates and nails will be used for the right humerus and ulna.
For the fracture of the lumbar vertebra, a prosthesis will be used as well as nails and rods
to support the weight.
For the left proximal phalanges, nails will be used.
