number 09066141917;

2. Aside from the mobile number 09066141917, I have other GCash accounts
which are no longer in use because I am not using those numbers;

3. I don’t personally cash in / top-up in my GCash wallet;

4. Instead, I receive money as payment from other Gcash accounts which

cause the top-up in my GCash wallet;

5. I usually cash out my GCash fund though online bank transfers;

6. This affidavit is hereby executed to attest to the truth of the facts above-
stated and for whatever legal purpose it may be used.
