This process outlines the enrollment of Optional Life Insurance and Critical Illness. There are two vendors that
support this Initiative for Heritage Worley. The vendors are Chubb Canada and Desj ardin Financial Security
(DFS). The map provides details of several sectors contributing in the process.

Medical evidence of insurability (Optional Life - Desjardins) - Required for amounts
over $50,000 (5 units) when applied for within 31 days of hire, during open enrolment
or a qualified life event. For all other times, required from 1st dollar of coverage.
Employee can apply for the optional benefits, and these are called Ancillary Benefits.
They can elect employee and/or spouse coverage. For completing the procedure, it is
essential to complete the application form and medical questionnaire, which is directly
sent to the insurer to assess for coverage.
Desjardins will send an email notification to CG to inform them to access the secure

employee's new benefit volume of coverage and new premium deduction via ADP GV.
• To update the details, we must click on P-30 master data.
• Next, we search for employee (personnel no) and insurance type and click on plan
overview.

• Then, we select the required or updated option from the list.
•
• After this, we edit the date and the units of the insurance coverage as per the mail
received and need to save it.

• We must find out the employee data with the plan and check out the data mentioned (Date, amount, insurance
plan) on the system.
•
• Chubb Canada:
• Optional Death and dismemberment: It do not require medical evidence but can apply for anytime. So, if they
apply for it, they'll send in the application and the data entry team just needs to add it.
• Optional critical illness benefit: If any one elect coverage over $100,000. A story and medical evidence must be
completed and then it needs to be added.
• Between the optional life and the optional critical illness, the employees can only apply within their 31 days from
the date of hire. So then, when the employee applies, they send the information to the insurer (Chubb Canada)
with the application and evidence for the insurability.
