Tarlac after having been duly sworn to in accordance with law, do hereby depose and say:

1. That I am applying in the Military Service as trainee in the Philippine Army;

2. That I was born on August 8, 1999 from the legal union of MYRNA BALA and
ROMEO BALA;

3. That I am still single, as I have never contracted any marriage with another person up
to the present;

4. That I have not sired a child with any woman;

5. That I am executing this affidavit to attest to the truth of the foregoing

statements and for whatever legal purpose this affidavit may duly serve.
