AFFIDAVIT
I, FRENCH KIMBER VALDUEZA, Filipino, of legal age, and a resident of Brgy. 74, Purok 6, Nula-
Tula, Tacloban City, Philippines under oath declare:

1. That I am the authorized representative of MECH AIR SYSTEMS CORPORATION by virtue of a

Special Power of Attorney executed before Atty. Rosalinda Montenegro registered under Document
No. 190, Page No. 38, Book 93, Series of 2022;

2. That MECH AIR SYSTEMS CORPORATION is the contractor for the Project: INSTALLATION OF
AIR CONDITIONING SYSTEM at Tacloban City Hall Extension Bldg.., Kanhuraw Hill, Tacloban
City, Philippines;

3. That said project has an accomplishment of 100% as of February 11, 2022 to March 8, 2022;

4. That all bills for labor and materials and other obligations necessary for the final accomplishment of
the project due to the government and any other person or entity, by reason of this contract are all paid,
likewise;

5. That I have complied with all the requirements for the minimum wage law and all laws pertaining
therein;

6. That I have not rented nor leased any government equipment for use in the aforementioned project;

7. That I am executing this Affidavit for the purpose of collecting the full payment due to the contractor
by virtue of the foregoing contract;

8. That I have not given nor offered gifts or monetary consideration to anybody or government personnel
who has to do with this Contract.

9. That this Affidavit sayeth no more.

IN WITNESS WHEREOF, I have hereunto set my hand this 14 th day of March 2022 in Tacloban City,
