I hereby declare that:

1. I am aware that the Aadhaar number provided on NSR portal of NSDL Database
Management Limited for opening of NSR profile will be used to download 'Aadhaar
Paperless Offline e-KYC' from UIDAI and I am willingly providing the Aadhaar number
for the same.

2. I am aware that the OTP received from UIDAI is required for downloading the 'Aadhaar
Paperless Offline e-KYC' and I am willingly providing the OTP on NSR portal.

3. I am aware that NSR has provided me a facility on its portal to download 'Aadhaar
Paperless Offline e-KYC' from UIDAI by entering Aadhaar number and OTP on its
portal. I'm willing providing my Aadhaar and OTP to NSR for further transmission to
UIDAI.

4. I hereby authorize NSDL Database Management Limited to use the information

downloaded from UIDAI to open the NSR profile.

5. I am aware that NSDL Database Management Limited, may share the information
submitted by me as a part of NSR profile creation, with NASSCOM, NSR member
companies, Empaneled Background Checkers (EBCs) and / or with relevant stakeholders.

6. I understand that NSR profile can also be opened with NSDL Database Management
Limited using other processes like submission of NSR Acknowledgement form,
biometrics and original photo identity proof document at NSR Point of Service (POS)
centre.

I hereby declare that:

1. I have downloaded the data from UIDAI using "Download E-Aadhaar / Aadhaar
Paperless Offline e-KYC" services.

2. I am willingly sharing the information as downloaded from UIDAI using "Download E-

Aadhaar / Aadhaar Paperless Offline e-KYC" with National Skills Registry (NSR) of
NSDL Database Management Limited.

3. I hereby provide my consent to create profile with of National Skills Registry (NSR) of
NSDL Database Management Limited using the information provided by me sought from
UIDAI using "Download E-Aadhaar / Aadhaar Paperless Offline e-KYC ".

4. I have not modified / changed the data before uploading it to create my NSR profile.

5. I am aware that NSDL Database Management Limited, may share the information
submitted by me as a part of NSR profile creation with NASSCOM, NSR member
companies, Empaneled Background Checkers (EBCs) and / or with relevant stakeholders.

6. I understand that NSR profile can also be opened with NSDL Database Management
Limited using other processes like submission of NSR Acknowledgement form,
biometrics and original photo identity proof document at NSR Point of Service (POS)
centre.
