PHOTO AND VIDEO CONSENT, ASSIGNMENT AND RELEASE

DECLARATION
Purpose: For marketing, advertising, promotional and/or communication purposes, Inventurus Knowledge
Solutions Private Limited ("IKS Health") may, from time to time, take and/or use photographs and/or video
recordings of IKS related activities or events that may include its personnel, whose photographs and video
recordings may be placed in the IKS PHOTOVIDEO BANK and which may be used by any associate and/or
affiliate entity or partner of IKS Health, worldwide (the "Purposes") for the Purpose set forth herein. For the
purpose of this form IKS Health may also include its subsidiaries, affiliates, assigns and/or successors in interest
as the case may be.

BY SUBSCRIBING THIS DECLARATION, I AM PROVIDING MY EXPRESS AND FREE CONSENT TO THE

TAKING OF MY PHOTOGRAPHS AND/OR VIDEO RECORDINGS BY IKS HEALTH FOR THE PURPOSE,
AND THAT I AM ASSIGNING TO IKS HEALTH MY RIGHTS IN FULL, AND FURTHER WAIVING ANY RIGHTS
THAT I MAY HAVE RELATED TO ANY SUCH PHOTOGRAPHS AND/OR VIDEO RECORDINGS, AND THAT I
AM CONSENTING TO THE USE OF ANY SUCH PHOTOGRAPHS AND/OR VIDEO RECORDINGS, IN
WHOLE OR IN PART, BY IKS HEALTH FOR THE PURPOSE.

For such sufficient and valuable consideration offered, the receipt and sufficiency has been expressly waived by
me without any undue influence or any promise of remuneration for the Purpose, I hereby agree to allow
photographs and/or video recordings to be taken of me, whether posed or candid, while I am on IKS Health
property and/or participating in IKS Health activities or events, to be used by IKS Health in connection with
the Purpose, whether on IKS Health's intranet, website, social media, digital media, magazines, newsletters,
indoor and outdoor branding related promotional material, printed materials or in any other medium (the "Work
Product"). I confirm that IKS Health will not be obligated to use the Work Product.

I understand that the Work Product is being created under the supervision, direction and control of IKS Health. I
hereby irrevocably assign to IKS Health any and all rights, including copyright, financial or moral, that I
may have in the Work Product. I agree that IKS Health has the sole worldwide ownership and rights in and to
the Work Product and its derivatives, including any and all copyright interests, and I acknowledge that I have no
interest or ownership in the Work Product or copyright. Photos and/or video recordings will not be sold to third
parties and will not be used by third parties except in cases where a third party has been contracted by IKS
Health to create the Work Product.

I agree that I will not bring or consent to others bringing a claim or action against IKS Health on the
grounds that anything contained in the Work Product or its derivatives, or in the manner in which the Work
Product is used, is defamatory, reflects adversely on me, or violates any other right whatsoever, including, rights
of privacy and publicity. I hereby release and forever discharge each of IKS Health and its officers, directors,
employees, agents, partners and affiliates and their respective heirs, executors, personal legal representatives,
successors and assigns, as applicable, from all actions, claims, causes of action, suits, demands, liabilities and
damages whatsoever, in law or equity, which I may have against any of them in connection with the Work
Product and/or its derivatives.

I confirm that I am competent to subscribe to this declaration and accord my unconditional and free consent to
use my photographs and/or video recordings purely for the Purpose and that I have not given anyone the
exclusive right to use my photographs and/or video recordings.

I acknowledge that I have carefully read and understood this Declaration and provide my free and
express consent to the same.
