Statement of Imputations of Misconduct or Misbehaviour on which action is proposed to be taken

Smt.V.K.Chitralekah ,MTS,Park Town HPO,Chennai-600 003.

Smt.V.K.Chitrlekha ,MTS,Park Town HPO, while working as MTS,Park Town HPO belated submission
of Medical Certificate from the following spells.

I. From 9.3.2021 to 13.3.2021

II. From 14.3.2021 to 20.3.2021

Smt.V.K.Chitralekah was infromede of her belated submission of medical certificate during the
above mentioned periods vide letter no H/01/dlgs dated 15.4.2021 issued by Head Post Master-HSG
I(NFG) Park Town HPO directing SMT.v.k.Chitralekah to submit the explanation for her belated
submission of medical certificate during the above mentioned periods,within a specified period and
they were sent by Registered letter number RT34305926IN booked at Park Town HPO which have
also been delivered to her on 16.4.2021.That the said Smt.V.K.Chitralekha ,MTS,has not so far such
belated submission of medical certificated,besides amounting to insubornation,amounts to lack of
devotion to duty and is also unbecoming of a Government Sservant.

Therefore,it is imputed that the said Smt.V.K.Chitralekha ,MTS,Park Town HPO failed to maiantain
absolute devotion to duty and acted in the manner which is unbecoming of a Government servant
Violating provisions of Rule 3(i) (ii) and (iii)of CCS (conduct)Rules 1964.

MEMORANDUM

Smt.V.K.Chitralekah ,Multi Tasking Staff,Park Town HO is hereby informed that it is proposed to take
action against him under Rule 16 of CCS (CCA)Rule,1965. A statement of the imputations or
misbehaviour on which action is proposed to be taken as mentioned above is enclosed.

Smt.V.K.Chitraalekha is hereby given an opportunity to make such rpresentation as she may wish to
make against the proposal.

If smt.V.K.Chiralekha fails to submit her representation within 10 days of the receipt of this
memorandum,it will be presumed that he has no representation to make and orders will be liable
passed against Smt.V.K.Chitralekha ex parte.

The receipt of this memorandum should be acknowledged by Smt.V.K.Chitralekha.
